Emergency Cards

It is extremely important to fill out and return the emergency card as soon as possible.  Accurate and current information will ensure your child’s safety.

Our Student Emergency Contact Cards serve two purposes.  First, the school needs to know how to contact you during school hours in an emergency.  We also need to know the names and phone numbers of at least two other adults who can act as parent substitutes, and provide transportation and care for your child in case of serious injury or illness.

Second, we will now use this card for the purpose of ensuring that your child is dismissed, for any reason, only to people you authorize.  Please list the names of all the people you authorize to pick up your child at school.

Please be sure to sign the back of the card.

Immunization Requirements – Several new laws have been passed and your child is required to have the following immunizations for entrance to 6th grade:

1. Tdap Vaccine — trade name Boostrix or Adacel. Some doctors prefer to wait until a child is 11 years old before giving them this vaccine. If your child is turning 11 before the start of 6th grade, he/she will need to provide proof of the vaccination to either the elementary or middle school nurse. Children turning 11 after school starts will need to be vaccinated within 2 weeks of their birthday.

2. Two Varicella Vaccines or one of the following:

1. A doctor’s note stating your child had chicken pox disease
2. A blood test proving your child had chicken pox.

3. For entrance into 7th Grade: On October 26, 2015, Governor Cuomo signed into law Chapter 401 section 2164 requiring 7th graders (and 12 graders) to receive the vaccine for meningococcal disease.

*The Rockland County Health Department offers vaccines for free for those who qualify and can be contacted at 845-364-2520 to make an appointment.

Medications In School – Any medication (including over the counter i.e. Advil, Tylenol) taken by a student during the school day requires a doctor’s prescription note with the name of the medication, dosage and frequency. For students with allergies, you can download files from the forms page.

Routine Physicals – Routine Physicals are required for grades: Pre Kindergarten or Kindergarten, 1, 3, 5, 7, 9, 11 and new entrants. If proof of a private physical is not submitted to the health office by October 30th, your child will be scheduled for an exam with the School Medical Officer.

 *If your child has had a school physical for a sport in 7th grade, that exam will satisfy this requirement.

Hearing and Vision Screening

Near and Far Vision Screening is required for the following grades: PreKindergarten or Kindergarten, 1, 3, 5, 7, 11 and new entrants, which includes near and far vision. New Entrants also require color screening in addition to the above.

Hearing screening is required for prekindergarten or kindergarten, 1, 3, 5, 7, 11 and new entrants.

Vision screening is done for the other grades when possible or requested. Screening usually starts in the Fall. You will be notified if your child does not meet the state minimum requirements for passing any of these exams.

Working Papers– Students obtain working paper forms from the Guidance Office and bring them to the nurse. If a student has submitted a private physical or has had a school physical within the past year, the nurse will complete the medical section of the form. If you do not have a current physical, the nurse can make arrangements for a physical with the school physician. Sports Physicals also meet the requirement. The student then needs to have his/her parent/guardian sign the form and bring the form to the guidance department which will then arrange for working papers.
